Home Care for those with various illness and physical limitations.

Are you able to maintain your independence in your home but need help with chores?  If you qualify, the State of Michigan Department of Health and Human Services will pay a caregiver to assist you in your home with non-medical needs.

Home Care Services Offered:

  • Meal Preparation and Cooking
  • Assistance with Errands and Shopping
  • Personal Grooming Assistance
  • Laundry Assistance
  • Light Housework and Cleaning
  • Incontinence Assistance
  • Care and Companionship
  • Medication Reminders
  • Respite Care to give caregivers a break.

Steps to get started:

  1. Call (248) 416-8903 and request a Home Care starter packet to be mailed to your home address.
  2. Make an appointment with your medical doctor and have your medical doctor complete the State of Michigan Medical Needs form. The purpose of this form justifies your need for service. This form will be mailed in the Home Care starter packet. 
  3. Call (248) 416-8903 once your Home Care Starter packet is complete. Four Seasons will take your paperwork to the Department of Health and Human Services. Depending on the County you reside, your application can be processed between 15 and 45 business days. 
  4. You will be assigned an Adult Service Worker with the Department of Health and Human Services. Please let the Adult Service Worker know that you are working with an approved agency called Four Seasons Housing and Care.  The services and hours you receive are determined by the State of Michigan Department of Human Services Adult Service Worker. 
  5. Once the State of Michigan Department of Health and Human Services Adult Services Division approve you for service, identify a relative or friend, that does NOT have a felony and does not mind providing services to you.  Have that individual call (248) 416-8903 to begin the onboarding and criminal background check process.
