Four Seasons Housing & Care was founded in 2016 by Dr. Jamita Lewis with a simple belief: every Michigan family deserves access to quality in-home care — regardless of income, language, or background.
We are a state-approved provider credentialed with Michigan Medicaid and multiple Community Health Plans of Michigan. That means we work directly with your MDHHS social worker, your health plan advocate, or your case manager to get your care approved and active.
We serve clients across Michigan — in English, Spanish, and Arabic — and we never leave a family to figure out the paperwork alone. It is easy to transfer to us — to Four Seasons Housing and Care.

Free eligibility call
About ten minutes on the phone. We confirm Medicaid or Community Health Plan status and ask what help your loved one needs day to day.

Paperwork, done together
We walk you line by line through the DHS-390 application and the MDHHS-6200 medical needs certification, then submit the packet on your behalf.

State review & approval
MDHHS or your health plan assigns a caseworker and schedules the in-home assessment. We follow up every week so your file never sits idle.

Training, then your first paycheck
You complete the required caregiver training, receive your certificate, and go on payroll for the care you were already giving.
